This program helps seniors experiencing Alzeimer's disease or other dementia, and those experiencing frailty or other age-related disabilities.

Adult Center

Caring and dedicated professionals help clients achieve their maximum level of independence and self-esteem.

Respite Care

Respite care allows primary care providers the opportunity to have a break from caregiving with the confidence that the older adult is being cared for by a trained provider.

  • Respite offers valuable time off from the daily demands of care and can decrease the stresses families face.
  • Care is provided on a short-term basis within the family or caregiver's home or within the community.
Chore Services

Chore Services provides eligible seniors with assistance in maintaining their homes.  To be eligible for the service, the recipient  must meet all of the following requirements:   1) must be over the age of 60; 2) the recipient or anyone else in the household must not be able to maintain a clean, sanitary, and safe home environment; and 3) any other relative or caregiver is not able, capable, or responsible for the provision of this service. Chore Services may include regular cleaning; washing floors, windows, and walls; snow shoveling, food/meal preparation and shopping and similar tasks. The service is offered under the Senior In-Home Services Program.  The grant pays for services to individuals who meet age and need criteria but do not meet the income and level of care criteria for Medicaid waiver services.  A minimal co-pay amount will be requested of the recipient based on monthly income. Inability to pay will not result in loss of service.

Care Coordination

The goal of the program is to enhance the lives of individuals and families living with dementia or other disabling illnesses. Care coordinators act as liaisons between families, individuals, and service providers.

Services can include arrangement for:

  • Information and referral
  • Personal care & Home Nursing
  • Home delivered meals & transportation
  • Home modifications
